- We welcome you to visit any other Neighborcare Health clinic.
- To make an appointment at another Neighborcare Health clinic or for questions about your care, call 206-548-5710. Our staff can assist you in finding dental, medical or behavioral health care.
- If you need emergency dental care between 12/13/2024 and 1/13/2025, call 206-548-5710.
- To get copies of your health records:
• Visit how to access your records on our website.
• Download, complete and sign a Release of Information form (see the link above to access forms) and take it to the clinic in person before 12/13/24.
For appointments:
- Please arrive 15 minutes before your scheduled appointment (30 minutes if it is your first appointment).
- If you are late, we may need to reschedule or may not be able to complete all the services scheduled for your appointment.
- If you are going to miss your appointment, please give us one day's notice so that we can reschedule.
