Registration
To use services at the health center when it opens, students must register with Neighborcare Health, even if they previously received are at the clinic. Parents/guardians can register their students, or students 18 and older can register themselves, by completing the Neighborcare Health at Ballard High School Registration Packet in DocuSign.
Students can use any of our services after a parent or guardian completes and signs the registration forms for the school-based health center.
Under state law, youth may independently access some health care without parental consent, including reproductive health and mental health services. Learn more about Washington state minor consent laws.
Registration forms are also now available at the school-based health center.
Mental health services at this health center are funded in part by the City of Seattle Human Services Department.
