Sports Physicals
Get ready for your sports team
When children and teens sign up to play sports with their school, with a sports organization outside of school, or to go to a camp, they are usually required to get a physical exam with a medical provider. Our school-based health center providers can see students before, during or after school for these physicals and take care of all the forms children and teens need to participate.
How to use our services
Complete a registration for the school-based health program. Find a school-based health center. Parents and guardians do not have to be present for a health visit but are welcome. We follow up with parents, as appropriate, after health visits. Registration packets are available in person at the school-based health center and online.
