Making healthy changes in your life is important and challenging. You do not have to do it alone. As part of Neighborcare Health's primary care services, your medical provider and behavioral health team may assist you with concerns such as:

  • Stopping tobacco use, such as chew, vaping or cigarettes

  • Discussing concerns about alcohol, drug, or substance use

  • Exploring medication options that may be appropriate as part of your primary care treatment plan, including medications used to address alcohol or opioid use disorder, such as Suboxone, Sublocade, Brixadi or Vivitrol

  • Helping you identify other resources outside of Neighborcare Health if your needs require specialty care

How to access services

If you are a current Neighborcare Health medical or dental patient interested in support for these health concerns, call or visit your clinic to make an appointment. Neighborcare Health can connect you to an integrated behavioral health team member and, when appropriate, to a medical provider to discuss medical evaluation, medication options, primary care treatment, or outside referral.

If you are not yet a Neighborcare Health patient, first establish care with Neighborcare Health. Neighborcare Health can then help determine what primary care, behavioral health support, or outside referral may be appropriate. Become a patient
