Chan Wu, AANP, MSN
Nurse Practitioner
Chan is a Family Nurse Practitioner who believes the best care happens when patients are truly involved in their health decisions. She works closely with each patient to create care plans that fit their needs and goals. Outside of work, she enjoys hiking, gardening, traveling with family and friends, reading novels, and exploring nature. She has led health workshops, tutored nursing students, and mentored new hires. She has also volunteered at the Homeless Care Center and the W.A.T.C.H. Mobile Medical Unit to care for people without insurance. These experiences have strengthened her belief that health care is about listening, partnership, and making a positive difference in people’s lives.

I chose Neighborcare’s school-based clinic because it brings care directly to students, making it easy for them to get the help they need without leaving school. I love connecting with students and their families, guiding them toward healthy choices, and seeing their confidence blossom as they grow stronger and take charge of their health.
