If you sign up for a MyChart account, you can

  • Communicate with your Neighborcare Health care team
  • See your medication list
  • Get lab results, vaccination records, and other health records
  • Request an appointment

How to request Medical Record Copies

There are three ways you can request copies of your medical records:

1. Request a copy of your medical records through MyChart

  • Navigate to My Record > Document Center and select: “View, Download, or Send Visit Records” or “Request Additional Records”
  • Please note: Dental X-rays and records prior to 2017 cannot be requested via MyChart.

2. Submit an online request for medical records via our secure website

Please note:

  • You may only request copies of your own medical records via the online portal. If you are requesting records for another patient (including your minor children), you will need to use option #3.
  • Dental records and X-rays cannot be requested via our online records request website. You will need to use option #3.
  • You will need to upload a copy of your driver’s license or government issued identification to access your medical records through our secure website.

3. Complete a Release of Information form

If you need your medical, dental or other health records from Neighborcare Health, please print and complete a Release of Information form.

Completed forms may be mailed, faxed or returned in-person to your clinic or to the Health Information Management team at:

Immunization records

If you need a copy of your own or your child's immunization (vaccination) record, you can print a copy directly from myirmobile.com. Go to the site to register for or log in to your account if you already have one. For more information on getting immunization records, visit the Washington State Department of Health's website.  

If you prefer to request your immunization record through Neighborcare Health, complete a Release of Information form and return it to Neighborcare Health. Your request will be processed within five business days.
