Event #1: The Power of Place-Based Care

  • WHEN: Saturday, May 2, from 10:30 a.m. to 1:00 p.m.
  • WHERE: The beautiful Panoramic Room at the Pacific Tower in Beacon Hill.
  • WHAT: Brunch and a robust discussion on the impact of place-based care, with a special focus on school-based health centers and services for those experiencing homelessness—followed by a chance to give and make an impact.

This brunch will bring together a powerful group of voices to show how place-based care transforms everyday lives. Opening remarks from Erin Dury and Tiffany Washington will ground the event in a shared vision—highlighting the consistency, collaboration, and financial support it takes to sustain this work across communities. The program will culminate in a panel conversation featuring Jennifer Becker, Health Center Coordinator; Zac Stowell, Principal of Robert Eagle Staff Middle School; Jen Beaton, RN Supervisor with the Health + Housing Outreach Team; and Steve Appleyard, Project Manager at The Morrison DESC. Together, they represent the full ecosystem of place-based care—schools, healthcare, and housing—and will speak to how this model removes unseen barriers, builds trust over time, and what becomes possible when community support helps sustain and grow this impact.

Event #2: Inside Whole-Person Integrated Care 

  • WHEN: Thursday, May 7 at 6:30 p.m to 8:30 p.m. (prompt start; please arrive by 6:30)
  • WHERE: Neighborcare Health at Meridian at Northgate in Seattle, WA
  • WHAT: A hands-on open house exploring how integrated care works across medical, dental, and behavioral health—featuring real stories, finger foods, and a chance to make an impact.

The evening is designed as an immersive experience, where guests engage directly with the impact of whole-person, place-based care from the moment they arrive. Through interactive elements like the Connected Care Map and Patient Care Journey, attendees step into what care looks like across schools, clinics, and housing—seeing how it all connects in real life. As the program begins, Candace will welcome guests and frame the purpose of the night, followed by keynote speaker Leanne Berge, CEO of Community Health Plan of Washington, who will speak to the power and outcomes of integrated care. The program then features voices from across Neighborcare—including Stephanie Furtado, Kristin Kurvink, Yi-Hui Chi, Tyler Lee, Alyssa Caucci, and Nate Kittle—each sharing how this model shows up in practice, before closing with a paddle raise led by Candice Parks.
