Including Neighborcare Health in Your Bequest

Specific bequest

"I give to Neighborcare Health (EIN #91-0893287), a Washington state non-profit corporation, currently located at 1200 12th Ave. South, Suite 901, Seattle WA 98144, or its successor organization, [specify dollar amount/specific property] to be used [for its general purposes]."

Residuary bequest

"I give to Neighborcare Health (EIN #91-0893287), a Washington state non-profit corporation, currently located at 1200 12th Ave. South, Suite 901, Seattle WA 98144, or its successor organization, [all or a percentage] of the rest, residue, and remainder of my estate to be used [for its general purposes]."

Contingent bequest

"If [primary beneficiary] does not survive me, then I give [specify dollar amount, describe specific property, or specify percentage of residue] to Neighborcare Health (EIN #91-0893287), a Washington state non-profit corporation, currently located at 1200 12th Ave. South, Suite 901, Seattle WA 98144, or its successor organization."
