Who can be seen?

Any student enrolled at Highland Park Roxhill Elementary can be seen by our providers by having a parent or guardian complete and sign the registration forms and return them to the school-based health center. They can either be delivered by the student or parent, or placed in a secure envelope and mailed to the health center's address (listed above). Printed copies of the registration forms are also available in the school-based health center. Under state law, youth may independently access some health care without parental consent, including reproductive health and mental health services.

More Information:

Before or after school appointments are available. Request to Report slips are given for appointments during school. We will do everything possible to schedule or reschedule so students miss as little class time as possible. Students may also visit the clinic without an appointment by getting a pass from their teacher or when referred by the school nurse.

When the school based health centers are closed, students are encouraged to call our High Point Medical and Dental Clinic, 206-461-6950. Neighborcare Health has additional clinic sites in the Seattle area where students are welcome as well.
